4 Paws for Ability primarily places service dogs with children. 4 Paws has a very small program for non-veteran adults seeking Diabetic Alert and Hearing Ear service dogs, who are fully independent and able to safely handle a service dog, and who are able to be placed with a small breed service dog.
There are three parts of the 4 Paws for Ability Service Dog application – the primary application, the health form, and two reference letter forms. Applicants must complete the primary application before they are able to have their health form and reference letters correctly filled out. You can begin your application and save it, if you need to return to it later. When you save the application you will be emailed a link to return to – please be sure you enter a valid email address, without this email you will be unable to access the saved application. After you have submitted your application, you will receive an email with a copy of your application and instructions to complete the health form and reference letters.
